Well this was an interesting issue that popped up. Normally in situations where an enemy becomes helpless - they die very shortly afterward and it doesn't much matter.
Lead-up to the event in our last Eberron game:
A baddie was knocked unconscious instead of killed (characters wanted some answers).
The characters proceeded to take all the time in the world to tie the baddie up good and well - this guy wasn't moving.
Baddie wakes up. Characters intimidate her.
More backstory - Baddie is a die-hard fanatic. Would rather kill herself than in any way help the party to attack her leader.
Baddie has an ability that has the description of: "The Blood of Vol Devote utters a word and the target is filled with wracking pain."
The Baddie was not gagged - due to the party attempting to get answers from her.
Now the question is - could said Baddie actually kill herself with her ability that apparently only needs her to speak a word.
So far the only thing I found on forums and Errata and other information sources is this: Helpless: Technically doesn't (by itself) prevent you from moving or attacking. Many writers may not know this though, so use your best judgment if it ever makes a difference (which should be rare).
I'll post more if I find it. I haven't given up yet.
